Assessment and management of pain in the palliative care is a very difficult process especially in the case of a non-verbal patient (Gambrell, 2005). Health-care providers play an active role in managing pain and providing comfort to such patients. But unmitig ated pain in the absence of adequate use of analgesics and sedatives can lead to enhanced morbidity and mortality in patients in the ICU (Ahlers et al., 2008). The tools available for pain assessment in noncommunicative patients are mostly subjective which is a great drawback. Ideally, an objective tool for quantification of pain intensity capable of providing a quick feedback would be needed to provide comfort to the unresponsive patient (Gambrell, 2005). “Raisin in the Sun” by Lorraine Hansberry Essay

Since the 1930’s, the idea that a family, a home, opportunity, money and security being available to everyone in the US has been the â€Å"American Dream.† Unfortunately, in reality this dream isn’t really available to everyone, not then and not now. The idea of an â€Å"American Dream† is examined throughout Lorraine Hansberry’s play, A Raisin in the Sun as the theme of the play surrounds itself around Langston Hughes’ poem, â€Å"Harlem† where Hughes examines if dreams shrivel and dry up like a â€Å"Raisin in the Sun.† Throughout the play, all the characters express their own dreams for their lives. The idea of the American dream is analyzed to the point that at the end of the play, although this dream isn’t available to everyone because of racial, gender and class discrimination, the most important aspect the Youngers find out of it is the uniting of the family. Every character in the book has their own idea of the American dream. Mama and Ruth dream of owning their own house and getting the family out of their current living situation while Beneatha dreams of getting an education, becoming a doctor and not being dependent on a man for anything. Walter, although he wants to support his family, has his dream of buying a liquor store to raise money for his family. He finds more pride in proving himself successful. Walter, although with a capitalistic way of thinking, sticks to his own dream and come off as a frustrated character throughout the play. He takes out his frustration about not having money on Ruth, â€Å"You tired, ain’t you?†¦So tired-moaning and groaning all the time, but you wouldn’t do anything to help, would you?† (32). At first his frustration is because of the family’s financial situation, but it just grows with Ruth’s pregnancy. Eventually, he realizes that his dream of the liquor store isn’t feasible when he actually has the insurance money and he deals with its loss. He realizes that money was not the only barrier keeping him from his dream. Walter learns that what everyone else is hoping foe, the home, is the ultimate goal he should also be supporting. Beneatha on the other hand, has the American dream of getting an education. During the time period of the play, not only was it unusual for a woman to go to medical school but also it was even more rare of a black woman. Beneatha faces many obstacles on her way to achieving her dream; she  constantly faces discrimination because of her race and class and especially because of her gender. Even with hard work and persistence, she would have a lot more difficult time achieving her dream. For example, for white males to be going to medical school at that time was very common. They did not have to face gender or racial discrimination. Beneatha, not only has to provide money for her education but also deal with society’s disapproval because of the path she chose to pursue. She doesn’t even get support from her own brother. Walter continuously belittles her dreams and says that she should â€Å"just get married and be quiet.† (38). Walter, along with society at that time, believed that a woman’s place was to stay at home, cook, clean and take care of the children. Although Mama and Ruth support Beneatha’s individualist attitude, they do not understand it. When Beneatha mentions that she is â€Å"not worried about who [she’s] going to marry yet-if [she] ever get[s] married,† (50) they are horrified at the thought of a respectable young woman not being married. However, as the play progresses, Beneatha realizes that the dream of owning a house is the dream of the whole family and that she must stand united with them against people like Carl Lindner. Throughout the play, Mama and Ruth’s dream of securing the family’s future through the idea of buying a house stays constant. They also face many obstacles in trying to achieve this idea in the â€Å"American Dream.† Carl Lindner, from the â€Å"welcoming committee† of the all-white Clybourne Park residence area, sees their race and tries to persuade and eventually bribe them into not purchasing their dream house. Mama, however, does not give into this obstacle and goes through with the purchase of the home. After Walter deals with the lost insurance money and Beneatha deals with the loss of money for her education, they start to accept the idea of uniting to pursue one dram for the whole family. They realize that the happiness that they get from fulfilling the dream of the entire family is much more important to them than fulfilling their own dreams. Individually, they all have many hurdles but when they stand united as a family, they can get through them all. Just as the Youngers faced obstacles on their path to the â€Å"American Dream,† most people in today’s society face similar problems. Whether it be the  dream of education, a house or opportunity, people face discrimination every step of the way. As much as the society would like to believe that everyone has equal opportunity to succeed in life, it is just not true. Race, gender and class discrimination creates barriers for people trying to achieve their dreams of success. Much improvement is needed in the mindset of society before everyone truly has equal opportunity to achieve his or her version of the â€Å"American Dream.†

History of the 19th Century Whaling Industry The 19th-century whaling industry was one of the most prominent businesses in America. Hundreds of ships setting out from ports, mostly in New England, roamed the globe, bringing back whale oil and other products made from whales. While American ships created a highly organized industry, the hunting of whales had ancient roots. It is believed that men began hunting whales as far back as the Neolithic Period, thousands of years ago. And throughout recorded history, the enormous mammals have been highly prized for the products they can provide. Oil obtained from a whale’s blubber has been used for both lighting and lubricating purposes, and the bones of the whale were used to make a variety of useful products. In the early 19th century, a typical American household might contain several items manufactured from whale products, such as candles or corsets made with whalebone stays. Common items which today might be made of plastic were fashioned of whalebone throughout the 1800s. Origins of Whaling Fleets The Basques, from present-day Spain, were going to sea to hunt and kill whales about a thousand years ago, and that appears to be the beginning of organized whaling. Whaling in the Arctic regions began about 1600 following the discovery of Spitzbergen, an island off the coast of Norway, by the Dutch explorer William Barents. Before long the British and Dutch were dispatching whaling fleets to the frozen waters, at times coming close to violent conflict over which country would control the valuable whaling grounds. The technique used by the British and Dutch fleets was to hunt by having the ships dispatch small boats rowed by teams of men. A harpoon attached to a heavy rope would be thrown into a whale, and when the whale was killed it would be towed to the ship and tied alongside. A grisly process, called cutting in, would then begin. The whale’s skin and blubber would be peeled off in long strips and boiled down to make whale oil. Whaling in America In the 1700s, American colonists began developing their own whale fishery (note: the term â€Å"fishery† was commonly used, though the whale, of course, is a mammal, not a fish). Islanders from Nantucket, who had taken to whaling because their soil was too poor for farming, killed their first sperm whale in 1712. That particular species of whale was highly prized. Not only did it have the blubber and bone found in other whales, but it possessed a unique substance called spermaceti, a waxy oil found in a mysterious organ in the massive head of the sperm whale. It is believed that the organ containing the spermaceti either aids in buoyancy or is somehow related to the acoustic signals whales send and receive. Whatever its purpose to the whale, spermaceti became greatly coveted by man.   By the late 1700s, this unusual oil was being used to make candles which were smokeless and odorless. Spermaceti candles were a vast improvement over the candles in use before that time, and they have been considered the best candles ever made, before or since. Spermaceti, as well as whale oil obtained from rendering the blubber of a whale, was also used to lubricate precision machine parts. In a sense, a 19th-century whaler regarded a whale as a swimming oil well. And the oil from whales, when used to lubricate machinery, made the industrial revolution possible. Rise of an Industry By the early 1800s, whaling ships from New England were setting out on very long voyages to the Pacific Ocean in search of sperm whales. Some of these voyages could last for years. A number of seaports in New England supported the whaling industry, but one town, New Bedford, Massachusetts, became known as the world’s center of whaling. Of the more than 700 whaling ships on the world’s oceans in the 1840s, more than 400 called New Bedford their home port. Wealthy whaling captains built large houses in the best neighborhoods, and New Bedford was known as The City that Lit the World. Life aboard a whaling ship was difficult and dangerous, yet the perilous work inspired thousands of men to leave their homes and risk their lives. Part of the attraction was the call of adventure. But there were also financial rewards. It was typical for a crew of a whaler to split the proceeds, with even the lowliest seaman getting a share of the profits. The world of whaling seemed to possess its own self-contained society, and one feature which is sometimes overlooked is that whaling captains were known to welcome men of diverse races. There were a number of black men who served on whaling ships, and even a black whaling captain, Absalom Boston of Nantucket. Whaling Lives On in Literature The Golden Age of American whaling extended into the 1850s, and what brought its demise was the invention of the oil well. With oil extracted from the ground being refined into kerosene for lamps, the demand for whale oil plummeted. And while whaling continued, as whalebone could still be used for a number of household products, the era of the great whaling ships faded into history. Whaling, with all its hardships and peculiar customs, was immortalized in the pages of Herman Melvilles classic novel Moby Dick. Melville himself had sailed on a whaling ship, the Acushnet, which left New Bedford in January 1841. While at sea Melville would have heard many tales of whaling, including reports of whales that attacked men. He would even have heard famous yarns of a malicious white whale known to cruise the waters of the South Pacific. And an immense amount of whaling knowledge, much of it quite accurate, some of it exaggerated, found its way into the pages of his masterpiece.

According to Joseph G. Morelli, MD, Rubinstein-Taybi Syndrome is characterized as, â€Å"a genetic disease that involves broad thumbs and toes, short stature, distinctive facial features, and varying degrees of intellectual disability† (2007). The condition itself is so rare that is occurs only about 1 in 125, 000 newborn babies (Rubinstein, 2012).The cause of Rubinstein-Taybi Syndrome can be attributed to a defect in the CREBBP and EP300 genes. The CREBBP gene provides instructions for making a protein that helps control the activity of many other genes. This protein, called CREB, plays an important role in regulating cell growth and division and is essential for normal fetal development. If just one copy of the CREBBP gene is deleted or change d, cells make only half of the normal amount of CREB binding protein (Rubinstein, 2012). The EP300 gene, unlike the CREBBP gene, controls protein development before and after the birth.In many of the cases of Rubenstein-Taybi Syndrome, no mutation is detected in either the CREBBP or the EP300 gene. In these cases, the case is unknown, but it is suspected that another unknown gene is responsible. Once the child begins schooling, the symptoms become slightly more obvious, demonstrating slow development of cognitive skills.Once a child begins to perform tasks such as drawing, coloring, and using scissors, teachers and parents will begin to notice slow development of motor skills accompanied by low muscle tone (Morelli, 2012). These symptoms are also a bit more noticeable because due to RTS, the thumbs and toes on an affected child will broaden beyond normal size, complicating everyday tasks. This will severely limit to what extent a child suffering from RTS will be able to perform in the classroom. However, it is unusual for people with the syndrome to become parents. There is no specific treatment for RTS that has been discovered as of yet. The Controversy of Euthanasia One of the biggest and most controversial topics throughout society today is the act of euthanasia in humans. In the medical field, euthanasia is commonly known as assisted suicide that is essentially for terminally ill patients only. When thinking about euthanasia, Americans tend to relate it towards the rights for animals, but in this specific example I will focus on the controversial topic of legalization on behalf of people who are professionally diagnosed with a life-threatening diseases. This will not include minorities under the age of eighteen or the elderly over the age of sixty.Thus when looking at the data in today’s society, euthanasia is clearly defined as taking action of ending a person’s life to relieve persistent and relentless pain. As of today, the majority of our nations population believes that euthanasia is immoral. Although euthanasia is illegal in the United States currently, some citizens argue in defense of dying p eaceful with dignity rather then suffer in a hospital bed for months on end. After several decades of consideration, euthanasia is extremely difficult task to break down due to both disagreements within the choice for and against this practice.For instance, the term ‘euthanasia’ comes from the Greek words eu meaning god, and thanatos meaning death (Manning 2). Voluntary is a medical classification of dying with consent from a patient within a reasonable amount of time before the termination process. Involuntary euthanasia is rarely seen today and is very uncommon due to new technologic advances in security and medical forensic sciences. In voluntary euthanasia, can although be simply defined as dying without consent (Nitschke). Under the English influence during the 15th century, active and passive euthanasia was categorized underneath voluntary medical practices.This is demonstrated in the process of the patient’s death. For example, active euthanasia is to end a person's life by use of drugs, whether by oneself or with the aid of a physician, when passive euthanasia is taking a persons life by not taking helping the patient survive during a ‘DNR’ circumstance, medically known as ‘do not resuscitate’ when need. Mattlin powerfully expresses that actively having the option to end ones life is unjust and should not be made legal. But modern medicine has not entirely solved the problem of terminal pain, and it certainly never will be able to answer the very personal question of an individual person's quality of life† (Humphrey 34). He claims his pro-euthanasia though ts through his groundbreaking novel about assisted suicide, called The Final Exit.Humphrey declares it is not right to say active euthanasia is ‘wrong’ in the sense of how passive euthanasia is morally adequate. He pushes his thoughts further by explaining that passive euthanasia is used in day-to-day cases and there is not anything we as individual citizens can do about it. This is because a doctor cannot force a terminally patient treatment if they aren’t willing to stay positive nor corporate. On a legal standpoint, Humphrey argues and debates the reasons of why active euthanasia is wrong due to the fact that many individuals refuse burdensome medical treatment in order as a legal and safe option of dying anyways.In addition, Derek Humphrey states that actively killing oneself is just the same as asking for physical medical assistance in order to take the life of someone with a fatally ill disease (Humphrey 16). Both active and passive voluntary euthanasia is so debatable due to medical realities shown in historical studies. For example, a man, commonly known as â€Å"Dr. Death†, or Jack Kevorkian, was an American pathologist, and euthanasia activist who is best known for publicly championing the aid of life-threatening patient's, on rights to die through physician-assisted suicide.On March 26, 1999, Dr. Kevorkian was charged with second-degree murder due to the clear statements and legal documentation of ending at least 130 of his patients to death in order to prove a point in his battling court case. After his conviction, Jack Kevorkian declares, on live television, â€Å"dying is not a crime† (Robinson). Soon following the dramatic loss in court, Kevorkian is constrained by reporters who publicly announce the federal reasons as to why he was sentenced to prison with second-degree murder by the court. Citizens of the U.S soon discover that sixty percent of the patients who committed suicide, with Dr. Jack Kevorkian's, hel p were not terminally ill, and thirteen had not complained of uncontrollable pain. The reports further emphasized that Kevorkian's counseling was too brief when deciding to euthanize a life. The public became officially aware of the horrid behind ‘Dr. Death’. The nineteen patients died within less than twenty-four hours after first meeting Kevorkian and evidence was found that he did not, in fact, preform a psychiatric exam in the nineteen cases he chose (Robinson).On behalf of Kevorkian’s terminated patients, five of individuals had severe histories of depression and Kevorkian was properly aware. Despite knowing the history of Kevorkians five individuals who were hopeless for reasons their medical condition, he still decided to illegall take their life.
